CRF First Choice Services

We offer many services for our clients to fit the varying needs and levels of support required for each individual. At CRF First Choice we strive to see our clients grow and reach their fullest potential, and we have developed many services and programs to help our clients in this process.

Behavioral Services

We provide many services and programs meant to address the various developmental deficits, maladaptive behaviors, and social/personal adjustment difficulties that many of our clients possess. We wish to help our clients integrate in with society in a healthy and positive way, and offering behavioral services serves to create a more stable environment for the individuals.

Community Residencies

Group homes and intermediate care facilities provide a community environment where individuals living in the residence receive habilitation, training, and supervision 24 hours a day based on the level of care of each individual’s needs. Community residencies provide each individual with the opportunity to create lasting relationships and help support themselves and those they live with.

Day Habilitation

Where residential habilitation is performed in home an often throughout all hours of the day, day habilitation is a short-term car provided for individuals who would normally have a family member or caretaker. Day habilitation can both happen at home or in a separate facility depending on the needs of the individual as well as the family, and it can also be offered as a way to integrate therapy into the individual's daily routine.

Home-Based Habilitation

Designed for individuals who reside with their own families or in their own homes, home-based habilitation focuses mostly on the development of abilities and social skills to support function in the family as well as in the community. The development of communication, socialization, and self-help are among the top priorities of home-based care.

Nursing

Nursing staff is available to all of our clients to provide medical supervision and support at any time. Nurses at CRF are stationed both in office and in our facilities and are available for care at all times. In any home, there will be 1-2 nurses on staff depending on the amount of care required and the number of residents within the home.

Personal Assistance

Even without a Medicaid waiver, we can offer in home assistance from trained staff for support with an individuals’ home maintenance, community participation, and skills enhancement. The aid of a personal assistance can also help with gaining and maintaining employment for an individual.

Residential Habilitation

CRF integrates outstanding care and services into the living arrangements of our clients who live outside of our own facilities. In addition to any personal assistance provided, our integrated living plan for residential habitation is guided through goals, objectives, and milestones.

Respite

For individuals who are unable to care for themselves alone but do have a family member or caretaker available most of them time, respite care services are available as a short-term care option while the primary care giver is not available. Respite care can be planned out and used regularly or simply used in case of emergencies where a caretaker is suddenly unable to assist an individual.

Semi-Independent Living

For those MR/DD individuals who are able to provide for themselves, CRF has semi-independent living facilities where minimal supervision is provided to allow clients to live in the community setting on their own. Individuals in semi-independent homes are responsible for maintaining both their own things and messes as well as house work and chores that are shared among the house.

Transportation

When attempting to get to and from work, appointments, and errands, it can be helpful to have and sometimes hinged on access to transportation. For this reason, we provide company vehicles and transportation to our clients to further help individuals in getting the support they require.

Waiver/Supported Living

We offer community home settings that our clients may live in if they qualify for support services (typically through a Medicaid waiver). In these homes, we provide support and care up to a full 24 hours a day. CRF will help to find each client the best living situation possible based on their income, development, and the amount of support required for daily responsibilities. While we have homes available for our clients to live in, if they can, we encourage that individuals stay with their families or in their own homes to maximize the amount of resources available to our clients.
